CHS-CRIF score
The CHS-CRIF score is a combined illness severity and dependency score, designed with reference to validated tools such as Frailty scores and modified Chronic Disease Scores.
The CHS-CRIF score will be used to enhance insight into the management and demographics of patients in the Virtual Ward environment and to support the development of enhanced clinical pathways and responsive workforce models.
Organisation Code: important for informatics for the relevant Organisation to be generated. If you would like a Code for your Organisation to enable informatics contact me dylan.jenkins1@nhs.net.
